You can add a sample statement form for members to use during an accident or incident.
Click on Emergency info link available in the left sidebar. The sub-links appears as below.
Click on Sample Statement Form link. The Sample Statement Form page appears where you have the text editor to add the content.
Use the text editor for basic text formatting as required.
Click on Save button to save the changes.
